YEP MENTORS.

YEP Mentors is a community-based mentoring program for young people ages 8 to 21 who have been referred by community partners. Youth are connected with a trained and dedicated Youth Advocate who provide supportive services, individualized goal setting, and case management.

Only youth referred by community partners are eligible for enrollment.

Services Provided:

  • Connect with youth 3 to 5 times a week

  • Help youth set and achieve individualized goals

  • Ensure access to basic needs and community resources

  • Coordinate care with family members, teachers, and other involved adults
